Hooiser Healthwise covers families who earn less than 200% of the Federal Poverty Level. Families
with income levels that exceed those listed below for Package C may still qualify but may be required to pay a minimal co-pay (maximum cost of
$24.75 per month). Please look below to determine your family's income level:
