This privacy statement covers the collection and use of Web site data for the Health and Hospital Corporation of Marion County (HHC), Marion County Health Department (MCHD) and Wishard Health Services (WHS).

Personally identifiable information
Personally identifiable information is specific information about visitors to our Web site. The personally identifiable information that we may collect when you use any of our sites include your name, mailing address, phone number(s), e-mail address and identification numbers.

We may request this information from you when you request a service, request informational materials or contact us. HHC, MCHD and WHS will release personally identifiable information about you as required to do so by law, subpoena, court order or legal process.

HHC, MCHD and WHS will not rent, sell or give away your personally identifiable information entered into their Web sites to third parties for marketing or mailing list purposes. However, your personally identifiable information may be shared as required by the Indiana Public Records Act.

We are required to maintain documents under the Indiana Access to Public Records Act, but we do not collect personal information other than to respond to you or for other purposes required by law.
