Objectives

Identify the unmet health needs of children through a variety of standardized health assessments during the summer.

Assist families in developing a "medical home" with a primary care provider to prevent illness and manage the treatment of identified health needs.

Work in coordination with Covering Kids and Families to enroll all uninsured, eligible children in Hoosier Healthwise.
