Asthma Screening
Children often times experience symptoms of allergies and asthma but have not been diagnosed. Under-diagnosis of allergies and asthma is a problem. Parents and children will answer questions regarding symptoms of allergies and asthma from the Brief Asthma and Allergy Screening survey.

Immunization Screening
If children have an immunization record located within the integrated computerized data system of the Marion County Health Department, the record will be printed and sent to the parents. The parents will also receive a copy of the latest immunization schedule. They can compare the child's immunization record and the record they may have at home to the immunization schedule to determine if their child needs additional immunizations.

Health Profile
The results of the health screenings and assessments are entered into a computerized database where a health profile is generated for each child. The health profile provides a brief description of the child's health status and any recommendations. Each parent/guardian receives a copy of the health profile. With parental permission, Project S.H.A.P.E. will send a copy of the health profile to the child's primary health care provider requesting it be reviewed and placed in the child's permanent medical record, along with instructions to follow up on any identified health needs.
