Who We Are/Mission
Begun in 2001, Project S.H.A.P.E. (Summer Health Assessment Program and Education) is a community
collaborative initiative in which the partners are committed to the principle that no child is left behind. All children deserve to have
the opportunity to reach their full potential physically, emotionally and cognitively through having and maintaining good health. This
opportunity is realized with professional medical and dental services. Our program, guided by the Healthy People 2010 objectives and
the American Academy of Pediatrics, promotes preventive health behaviors, skills and education for families with children ages 2
through 17.
As a broad-based community partnership that is housed within Health and Hospital Corporation of Marion County, Project S.H.A.P.E.'s
program model is congruent with the national campaign for 100 percent access and zero health disparities for low-income children. This
campaign is where a community declares its desired outcomes, engages its key partners and aligns its assets to ensure the
program's success.
Our Mission
Project S.H.A.P.E. seeks to improve access to health care for low-income children through detecting unmet health needs and ensuring
those children with such needs connect with a medical home where they can receive the appropriate health care services.
